A 24-bed Transitional Care Unit, for patients in need of short-term rehabilitation care not exceeding 21 days, is now open at Helen Hayes Hospital, located in West Haverstraw, NY.

The unit is available for patients recovering from knee and hip replacement surgery, and fractures and other orthopedic conditions, as well as for those who are physically deconditioned due to surgery, medical complications, or other disorders.

Patients must be admitted directly from an acute care hospital, where they have had a minimum 3-night stay, according to a media release from Helen Hayes Hospital.

Customized treatment plans for the patients could include physical, occupational, and speech therapy; 24/7 on-site physician care; rehabilitation nursing; recreational therapy; pain management; and medication evaluation and optimization.

Treatment goals could include may include independent stair climbing, managing daily routines at home, and regaining strength and endurance, per the release.

“The new Transitional Care Unit is an ideal option for individuals who can benefit from intensive rehabilitation, with the goal of returning home at an improved level of functioning and mobility,” says Patricia LeGeyt, BSN, chief nursing officer, in the release.
