Skilled nursing facility Guardian Rehabilitation Hospital, in collaboration with in-home care company 24Hr HomeCare, bring forth Continuum Healthcare, a non-medical in-home care division to extend patient care into the home.

“As a preferred partner of local hospitals in the Beverly Hills and West LA area, we have a responsibility to not only provide great care within Guardian’s walls, but also ensure that our patients transition successfully back to their residence,” says Guardian’s Vice President of Business Development, Tali Regev, in a media release.

“24Hr HomeCare has a stellar reputation in the community and has shown a commitment to innovation and positive patient outcomes. With our collaboration, we will be able to more clearly communicate instructions to caregivers. Continuum also allows us to serve patients that may go directly home from their acute stay as there is not a requirement to be admitted to Guardian first,” Regev adds.

With this collaboration, 24Hr HomeCare will operate the division and Guardian will oversee the program and provide strategic guidance.
