As a result of this roll-out, Munson reports that its readmissions rate has reduced from 12.8% to 4.5% in just 30 days for patients with COPD and CHF as primary diagnoses – a reduction of 64%.
Other improvements during a 30-day time frame include:
- Reduction in its post 30-day readmission rate from 11.8% to 3%
- Readmission rate of 0% for coronary artery bypass surgery patients
- Better than 95% patient satisfaction rate

Following this roll-out, Munson is expanding its remote patient monitoring program by doubling the number of kits from Vivify Health.
