Vivify Health announces it has partnered with Munson Healthcare Home Health in the successful implementation of Vivify’s remote patient monitoring technology.

As a result of this roll-out, Munson reports that its readmissions rate has reduced from 12.8% to 4.5% in just 30 days for patients with COPD and CHF as primary diagnoses – a reduction of 64%.

Other improvements during a 30-day time frame include:

  • Reduction in its post 30-day readmission rate from 11.8% to 3%
  • Readmission rate of 0% for coronary artery bypass surgery patients
  • Better than 95% patient satisfaction rate

Following this roll-out, Munson is expanding its remote patient monitoring program by doubling the number of kits from Vivify Health.

Munson Healthcare Home Health is part of Traverse City, Mich-based Munson Healthcare. The system includes nine community hospitals and related organizations that serve people in 30 counties. Munson Home Health serves 23 of those counties.
