The new partnership offers continuing care for those in fragile health so they can make a smooth transition back home after a stay in a hospital or in a skilled nursing facility. The new HomeCare Maryland extends the reach of LifeBridge Health’s services to make it convenient to people who need continuing care.

HomeCare Maryland provides a range of specialists who treat and care for patients in their own homes, according to instructions from their own physicians. These experts include physical and occupational therapists, speech pathologists, nurses, home health aides, social workers, and dietitians.
