PreparedHealth announces its partnership with Homewatch CareGivers and enTouch—a digital network that helps streamline care for patients from hospital to home.

Home care provider Homewatch CareGivers provides services via more than 190 locations in seven countries and 33 states. The enTouch partnership will provide greater visibility into patient care in the home for their partners, including hospitals, skilled nursing, and home health, according to a media release from PreparedHealth.

Via enTouch, everyone involved in a patient’s post-acute care stays connected in real time, receiving HIPAA-compliant and sometimes life-saving care updates and automated alerts for intervention and to help ensure that the patient’s in-home care will be a success.
