Select Medical Corporation and Alternate Solutions Health Network (ASHN) announce Select At Home, a joint venture to provide home health and hospice services to its patients and families in select markets nationwide.
The new collaboration, which marks Select Medical’s nationwide entrance into the home health care and hospice market, aims to extend the recovery and care continuum post-discharge from Select Medical’s specialty hospitals by delivering care for patients recovering or facing life-limiting illness at home.
“Our collective goal is to set the bar for exceptional, quality home care for the growing patient population that is in need of extended post-acute care services,” says Select Medical President and CEO David S. Chernow, in a media release.
“Select At Home will allow us to address a significant need raised by our premier health care system joint venture partners across the US who look to extend their unmatched hospital-based medical rehabilitation and critical illness recovery care into the home,” he adds.
ASHN partners with health systems nationwide to integrate care and services in the comfort of the home to support the value based and population health initiatives of each health system.
